ABSTRACT

Objective To study the clinical characteristics and prognosis of cerebral infarction combined with metabolic syndrome.Methods From July 1,2015 to March 31,2017,250 patients with cerebral infarction combined with metabolic syndrome in four comprehensive hospital in Zhongshan city were studied.The blood pressure,blood lipid,blood sugar,waist circumference,neurological deficit and disability were analyzed before and after treatment. Meanwhile,the neurological function and disability degree of blood pressure,blood lipid,blood sugar,waist circumfer-ence and non qualified patients were compared.Results After treatment for two weeks,three months and six months, the systolic blood pressure [(132.21±11.25)mmHg,(126.37 ±10.26)mmHg,(122.29 ±11.12)mmHg], diastolic blood pressure [(90.12 ±5.21)mmHg,(84.16 ±4.11)mmHg,(82.23 ±3.10)mmHg],triglycerides [(1.81±0.24)mmol/L,(1.64 ±0.35)mmol/L,(1.51±0.25)mmol/L],high -density lipoprotein cholesterol [(1.01±0.31)mmol/L,(1.08 ±0.36 )mmol/L,(1.19 ±0.34 )mmol/L],fasting plasma glucose [(6.26 ± 0.56)mmol/L,(6.10 ±0.34)mmol/L,(5.59 ±0.34)mmol/L],two-hour postprandial blood glucose [(8.36 ± 1.12)mmol/L,(7.34 ±1.10)mmol/L,(7.05 ±0.26)mmol/L],neurological deficit scores [(31.26 ±1.16)points, (26.58 ±1.05)points,(21.10 ±0.34)points],disability scores [(3.26 ±0.34)points,(2.10 ±0.25)points, (0.59 ±0.20)points]were better than those before treatment (F=5.10,5.23,5.45,4.89,4.21,4.20,4.26,4.58, P=0.02,0.04,0.02,0.03,0.01,0.02,0.02,0.03).After treatment for three months and six months,the waist circ-umference [(82.21±5.10)cm,(81.23 ±4.16)cm]were less than before treatment (F=5.26,P=0.03).After treatment,the compliance rate of blood pressure,blood lipids,blood glucose,waist circumference was 79.20%,and the not met rate was 20.80%.The neurological deficit score [(17.25 ±1.26)points]and disability score[(0.34 ± 0.12)points]were lower than those who did not meet standard(t=29.64,17.44,P=0.01,0.01).Conclusion High blood pressure,high blood lipid and high blood sugar are the main clinical characteristics of patients with cerebral infarction combined with metabolic syndrome,and relieving the symptoms of"three high"can improve the prognosis.

ABSTRACT

Objective To investigate mental health literacy (MHL) of community medical staffs in district and town of Zhongshan city,and to provide data to improve MHL of community medical staffs.Methods Totally 352 medical staffs who were not psychiatric physician and 81 psychiatric physician were selected.The Chinese Mental Health Knowledge Awareness Questionnaire (published by Ministry of Health) was used to investigate the awareness rate of mental health knowledge.Five vignettes of two schizophrenia,one bipolar disorder,one depression and one obsessive compulsive disorder,each with 8 related questions,were used to investigate the recognition and response ability.Results The average awareness rate of the two groups was more than 80.0% (87.6%,91.6%).For the item 2(75.6%,88.6%),6(74.1%,62.6%),19(36.5%,65.6%),and 20(74.1%,86.2%),the awareness rates were lower than 80.0%,and there were significant differences between the two groups (x2=8.45,4.92,27.48,6.99,all P<0.05).In vignettes survey,the correct rate was lower in the staffs who did not engaged in the mental health work than those in the other group,the difference was statistically significant(64.6% vs.75.9%,P<0.001).For both of the two groups the correct rate of depression was the lowest(x2=44.46,P<0.001).There was statistically significant difference between the total (x2=141.17,P<0.001).Conclusion The awareness rate of mental health knowledge has reached the national standards for community medical staffs,but they have to improve for some knowledge point.Their recognition and response ability for mental illness should be improved.

ABSTRACT

Objective To compare the timeliness, success and mortality rates between the modified carotid artery puncture method ( MCAPM) and standard suture method ( SSM) in the establishment of rat model of a middle cerebral ar?tery occlusion ( MCAO) . Methods Thirty?two male rats were randomly and equally assigned into MCAPM group and SSM group. The MCAO models were established by inserting a thread into the common carotid artery ( CCA) . 24 h after modeling, the rats of the two groups were evaluated with Bederson neurological scores, and the modeling success rate and mortality rate were analyzed. Results The suture insertion times, success rates and mortality rates of the MCAPM vs. SSM groups were (82?3 ±17?4) s versus (164?6 ± 22?0) s (P0?05), and 6?25% versus 18?75% (P>0?05). Conclusions MCAPM can be used to establish the rat model of MCAO due to its simplicity, mild wound and feasibility.

ABSTRACT

Objective To investigate the factors that induce constipation in bedridden patients undergoing replantation of severed finger and find countermeasures. Methods A retrospective investigation was carried out in 92 patients undergoing replantation of severed finger, their constipation and factors influencing constipation were recorded. Results 61 patients suffered from constipation in total 92 cases,the rate being 66.30%. 76 patients reduced food intake, and 59 cases presented constipation (the rate being 77.63%). Factors influencing constipation included being unaccustomed to defecate in bed (the rate that present with constipation being 87.09%),reluctant to trouble others(75.47%), side effects of drugs(55.56%) and lack of dietary knowledge (71.87%), decreasing food intake was closely bound up with constipation. Conclusions Proper psychological counseling, strengthening of health propaganda, comfortable facility for defecation, improved family support and oral food intake can significantly reduce the proportion of constipation and increase comfort degree of patients.

ABSTRACT

OBJECTIVE@#To evaluate the clinical application value of nasal airway obstruction objectively and quantificationally by acoustic rhinometry (AR) in children with adenoid hypertrophy.@*METHOD@#Thirty patients with adenoid hypertrophy were examined with AR and questionnaire at preoperative and eight weeks after surgery. The values of AR were compared with those of 27 normal controls.@*RESULT@#There were significant differences between preoperative and postoperative in NAR and NPV (P 0.05); There were significant differences in NAR, NPV, MCSA between the patient group and the control group (P 0.05); There was negative correlation between MCSA and symptom scores (r = -0. 519, P 0.05).@*CONCLUSION@#Acoustic rhinometry was an objective and quantitative method for evaluating nasal airway in children with adenoid hypertrophy. In interpretation the relationships of the results of acoustic rhinometry reflection measurements and subjective symptoms, MCSA should be the main reference.

ABSTRACT

BACKGROUND: Early clinical researches suggest that mediodespidine capsule can decrease blood pressure of patients with primary hypertension on the basis of yanggan qinggan pinggan technique.OBJECTIVE: To observe the effect of mediodespidine capsule on primary hypertension and angiotensin Ⅱ (Ang- Ⅱ ) of patients in the types of yin deficiency, yang excess and liver fire.DESIGN: Randomized controlled case observation.SETTING: Department of Cardiology, First Affiliated Hospital, Hunan University of Traditional Chinese Medicine.PARTICIPANTS: Patients were selected from the Inpatient Department and Out-patients Clinic of the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine between April 1999 and April 2000. Inclusion criteria: Patients, who were diagnosed with the criteria of primary hypertension, who belonged to the types of yin deficiency, yang excess and liver fire, who could endure this test, who were coincidence with the criteria of mental-stress test, were all consent. Exclusion criteria: Patients who had severe congestive heart failure, atrophic arthritis and unable tolerance were excluded. There were 60 cases in total, including 29 males and 31females. All patients were divided into mediodespidine group and verapamil group with 30 in each group on the basis of randomly digital table.METHODS: Patients who had positive results of awaken mental-stress test were involved in this study. Cases in mediodespidine group were treated with mediodespidine capsule, which was made of tall gastrodia tuber, spine date seed and fourstamen stephania root at the dosage of 0.43 g/pill and were provided by the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine (wei yao ji zi: 05-047, batch number: 990503), five pills each for 3 times a day. Cases in verapamil group were treatedwith verapamil (40 mg/pill, Shanghai Huanghe Liya PharmaceuticalCompany Limited,batch number: 980301B) 80 mg each for 3 times a day. The course in both groups was 4 weeks. ① Effect of blood pressure: Average value of blood pressure tested for 3 successive days were regarded as the blood pressure before treatment, and average value of blood pressure tested for 3 times after treatment was regarded as the blood pressure after treatment. ② Effect of symptom:Scores were recorded as four levels according to main symptoms, such as vertigo, headache, pain and weakness of waist and knees, before and after treatment. ③ Values of Ang- Ⅱ were measured with radioimmunoassay before and after cold pressor test. Effect on Ang- Ⅱ was evaluated with effect of decompression, effect of clinical symptom and criteria of clinical effect.MAIN OUTCOME MEARUSES: Clinical symptoms, values of blood pressure, heart rate and Ang- Ⅱ before and after cold pressor test.RESULTS: A total of 60 patients with primary hypertension were involved into the final analysis.① Totally effective rate of clinical symptoms of patients in mediodespidine group was higher than that of patients in verapamil group, but the effect of decompression was not significant between the two groups (P > 0.05). ② There was remarkably significant difference of heart rate in both groups before and after treatment (P < 0.01), especially the decrease in verapamil group was great, and there was significant difference (P < 0.05). ③ Values of systolic pressure and diastolic pressure were decreased in both groups after cold pressor test (P < 0.01), but there was no significant difference between the two groups (P > 0.05). ④ There was no significant difference in clinical effect between the twogroups (P > 0.05).⑤ After treatment, concentration of Ang- Ⅱ at basic level in both groups was decreased, and there was significant difference between them (P < 0.01);concentration of Aug-Ⅱ was decreased in both groups after cold pressor test as compared with that before treatment, but there was no significant difference between them (P > 0.05).CONCLUSION: Mediodespidine capsule can decrease blood pressure stably, and the effect is stronger than that of verapamil at the aspect of decompression and improving symptoms. Blood pressure of patients with primary hypertension in the types of yin deficiency and yang excess or liver fire can be decreased by decreasing the level of plasma Ang-Ⅱ to a certain degree.

ABSTRACT

@#ObjectiveTo investigate the tossing and turning time of the stuporous patients while using the bedsore mat blowing out gas.Methods45 cases were as the control group and treated with routine nursing. 117 cases were as the test group, in them, 17 cases were tossed and turned once every 2 hours, 50 cases tossed and turned once every 4 hours and 50 cases tossed and turned once every 6 hours. The patients of the test group were nursed with the bedsore mat blowing out gas as well as routine nursing. The rates of bedsore of two groups were compared.ResultsThe bedsore rate of the control group was 4.44%, 2 hours group was 0%, 4 hours group was 4% and 6 hours group was 22%; there was no significant difference among the control group, 2 hours group and 4 hours group, but there was a significant difference between 2 hours group, 4 hours group and 6 hours group (P<0.01).Conclusion4 hours is suitable for the stuporous patients to toss and turn while using the bedsore mat blowing out gas.
